I don't think I posted my reply message properly yesterday.
I still don't know the answers to many q's either because research was difficult, despite Woods and Woods-Campbell being around the longest. Previous patients have not been very active on these websites.
I'd rather not post price details for public viewing because pricing varies depending on individual patient complexities and total no. of grafts (price per graft reduces as no. of grafts increases). Dr Woods can give you indicative estimate if you email him your photos.
