    Default Chicken or the egg?

My doctor says my red, flaky, itchy scalp is causing my telogen effluvium (I have mpb too), but I've read that high dht levels cause a red, flaky, itchy scalp. So which is it? Scalp issues causing hair loss or hair loss causing scalp issues?

A scalp biopsy by the dermatologist will give you more definitive answer or diagnosis to your scalp condition.
