My friend, don't be afraid to travel. Don't let logistics be a limiting factor on finding the best doctor for you. Most reputable clinics offer a travel reimbursement.
Look into Dr. Wong or Dr. Hasson of Hasson&Wong. They are located in Vancouver Canada. I have done a lot of research and they seem to be the best around. I live in CA and am planning on doing my procedure with Dr. Wong.
I had a previous procedure with a local surgeon in San Diego (i too was looking only at local doctors), and that was a nightmare for which i am still paying the price.
What are you looking at doing? strip or FUE?
From your pic, it looks like you need a significant amount of transplants, for which strip is the best option.

I live in memphis and I went to Dr. Jack Fisher located in Nashville. He is an IAHRS doctor. He did a pretty quick scan by just looking at me and said he could do 600 grafts. I decided 600 grafts would not be enough to show any improvements. He said if he were to do anymore,I was in for some shockloss.
Goto Nashville and get a free in person consultation from him. I would be bery curious as to what he says. He has a good reputation. Hey its free man ,it can't hurt and he is an IAHRS certified doc.

Hi,
Intially my advise ( and i am NOT a doctor) is stand fast, wait. Looking at the pics on the link you are in store for further loss unless to attempt to halt it via the meds. You want to make sure your loss is stable prior to embarking down any HT road. Even then take your time and be sure its for you given the risks involved.
Look into Minoxidil, Propecia (proscr ) and nizoral shampoo - "The Big 3"

The big 3 to beat DHT, THE CAUSE OF HAIRLOSS!
- Propecia ( Finasteride ), Minoxidil and Nizoral
Here is the thing you need to make sure you understand when it comes to MPB:
The main known factor that causes hairloss is DHT. To form DHT your body's 5a-reductase enzyme converts free roaming testosterone DHT inside the hair cell follicle. The DHT then binds to the hair cell receptor and causes it to atrophy or shrink. Eventually your hair follicle stops producing hair and you lose that follicle.
There are two types of the 5ar enzyme; Type I and Type II. Type II 5ar seems to be the main culprit in MPB as the type II enzyme is present in the hair follicles. Finasteride is a specific inhibitor of Type II 5ar inhibitor that reduces DHT produced by the type II 5ar by 85%-90% and reduces the overall DHT blood serum levels by 65%.
