Looks like you have some diffuse thinning going on there, extended from the crown just past the mid top of.

You still have some live follicles in the crown area but does seem sparse. I recommend seeing a dermatologist and probably getting on fin + minox. Be wary of the prescribing information before taking fin.

Could also try some supplements like HR23+ which many have said has given them improvements or even groMD, both in addition to the fin + minox.

In terms of how long you have to go, no one can tell you this. Hair loss can take years to go from one phase to the next, or be rapid. Some people may recede and have small thinning at crown and then never experience any further loss all the way up until they reach the grave. Overall, the aim of the game is to engage in healthy lifestyle habits to slow down the progression.
