    Makes a lot of sense. Are there any advantages to using a bigger punch at all? Why use them if there is no benefit ?

    Every patient is different. I have a .85 Cole Instruments punch I took photos of sitting on my desk. That is one tiny tip. Donor areas look great after just a couple days when the smallest punch is used. We commonly see the extraction sites virtually disappear after 5 days when ACell and PRP are administered.

    The photo below is an example of a follicular unit type that would be transected if a .85 punch were used. When a patients follicles run parallel within a follicular unit, a .85 punch can usually be utilized to score part of that group, leaving hairs behind and leaving a fuller appearing donor area.

    There is a little bit more fat to protect the graft and also logic say that less risk of transection (damage of the graft during the extraction). I don't really care about the tiny difference between .8mm vs 1mm in the "scar". I don't think anybody will notice even if you have short hair.

    You need a variety of options to be successful with FUE. There is no such thing as one size or one technique suits all patients. With some patients, the follicle damage (transection) rate will be over 20% with a 0.8 mm punch. Many of these damaged follicles will simply die. That's fine if you are headed to a NW 2 or 3, but if you are heading to a NW 5 or 6, you need to consider options that insure the survival of every hair. The approach to FUE is far more complex than suggested here, in my opinion.
