Wages are also a fraction of what they are in the West.

Why is it any different using your Western money to get a hair transplant for a fraction of the cost for the same procedure in the same way that you could buy up a few cheap properties in poorer countries to rent out etc? Also India and Pakistan, though poor are not Somalia or some rogue Mad max states. They have laws in place as well, in fact the same Common Law system that you have in the United States. So why do you think that they would get away with been butchers?
Most US HT doctors have been butchers tbh....
Just seems like a lawsuit against a Pakistani or Indian surgeon would be even harder than against a doc in your own country

As a pakistani let me assure you that getting medical treatments in pakistan is a very, very bad idea.

To you it is a waste of money. To others it is money well spent. When shopping for a hair transplant surgeon, making a decision based primarily on price is generally not a very good idea. There are many other factors to consider that are likely to be much more important than the price.

Speaking for myself, there is absolutely no way I would go to India or similar countries for any surgery of any kind. It is much too risky. The risk outweighs the savings by a very large margin.

Something is wrong if you talk in terms of continents instead of specific physicians.

A hair transplant is not just a surgical procedure, but cosmetic and therefore requires proven artistic competence if you want to get, say, the hairline, exactly right. You can only be sure of your result if you research specific doctors and their portfolio of patients. It's irrelevant where they're from, but the vast majority of the few good ones come from Western countries, like Belgium and North America.
