Is there any data on how successful hair transplants by IAHRS surgeons are?
What are the chances of a hair transplant failing?
Have any IAHRS surgeons worked on someone who had little to no re-growth?

I doubt there is any available data on "success" rates of any hair transplant surgeon IAHRS or non-IAHRS. Success in many cases is subjective and with today's techniques a total failure (no growth, bad scars etc.) is probably very rare even in places that were once notorious for doing bad hair transplants.
I think the best way to go into it is to understand that anything can happen even in the most capable hands. If you want guarantees, don't have a hair transplant. I think the only thing that is guaranteed by going to an IAHRS surgeon is that they are capable of performing insanely good hair transplants compared to non-IAHRS surgeons, but it does not guarantee that you will get one.

It is the most worrying thing about it.......especially FUT.
Paying thousands of pounds and coming out worse than you went in, with a massive scar across your head.

That's not true at all -- clinics could definitely track success in terms of number of transplanted grafts that grow, hair caliber of grafts before/after transplant, average number of hairs per FU before/after transplant, etc.
They'd rather not do that for obvious reasons ... far simpler to make vague claims like "complications are very rare", or "your expectations were unrealistic".
