For permanent SMP, you closest reliable clinic would be HIS in Chicago. For temporary SMP, your closest would be Shapiro Medical Group in Minneapolis.

HIS is not that great.. I've read lots of posts on their boards of turning blue, looking fake, and fading.. Their a huge company and spread far too thin with way too many different practitioners to get a guaranteed result.
