    Dr. Lindsey works on a tough scar McLean, Va
    This nice fellow has been presented in a previous before/after 3000+graft case by me. 8 month check on 3200 graft frontal revision by Dr. Lindsey McLean VA

    He had gone to a big chain hair operation several years back and got 2000 grafts with little to show for it except a pretty big scar. We did a large case for him and got great results last year. In that case-story I explained how we took our strip up to but not including is old scar, which was a pretty bad scar to begin with, as the patient wanted the most hair we could do, and felt he'd come back later for the scar.

    Well we excised his scar about a month ago. Shown are preop pics illustrating recurring granulomas on the right side which I felt were not likely from sutures but rather from ingrown hairs. I tried steroid injections but they were not successful. Also shown is his nice 2 layer, everted, tension free closure. Sutures came out at a week. 2 weeks later I had him stop by for a check, and he said things felt fine. Interestingly, he said he spent alot of time squeezing his scar after his first case elsewhere and my case a year a nd a half ago. I told him DON'T do that. He's using vitamin E oil and will be back again for a check next week.

    We are not out of the woods yet, but so far, so good. Time will tell. With scar repair, there is no guarantee that you will get a better reuslt. You simply try your best on an educated patient; and it more often than not reusults in a significant improvment.

    An interesting update on this fellow's scar. Attached are 6 week pictures. He is doing well. He called me and said that his primary care doctor had started treating him for folliculitis on his extremities. Added in the history is that almost daily he spends an hour in a hot tub after exercising at a gym. I suspect that is where he got the extremity folliculitis, and I bet that played a roll in his scar issue previously and possibly even in this recovery phase.



    Shown are his current pictures, and I am now hopeful that treating his systemic folliculitis and avoiding the hot tub, which I hadn't asked about, will allow him to get a nice scar repair.
