All healthcare professionals in Hong Kong have to complete this form prior to being granted some other medical treatment. The digital health declaration type was introduced in 2021 included in the medical reform policy by the authorities of Hong Kong. Currently, it is one of the most frequently completed forms in Hong Kong.
